Too crowded specially on weekends. Nothing special considering the high volume of local visitors! Better to enjoy this monument from distance specially when there is no smog is around !

No need to book. Best time depends on weather, whether there is visibility or not and if you like day or night. Night is magic in Shanghai but you don't need to be high to enjoy the night skyline.

You do not need to buy online. CAn do it when you get there. Easy. No need to buy the upper part of the tower. Yes the glass walk is included as part of the ticket. Enjoy
