When we visited Bali and stayed at the Prima Cottage, we simply took a taxi from the airport to the hotel. Simply be sure to have the address written down to help the driver find it. The cost of the taxi is preset at the airport taxi counter. There are also a lot of freelance drivers who will provide transportation for a slightly lower price.
I had made sure that I had enough cash in local currency to pay for the trip when I arrived. It is recommended to have "small" bills on hand : a few 10,000's for example (equivalent to US$1.00) as well as 50 and 100 000 (the largest bill - US$10).
When going east (to Sanur or Ubud, for example), the driver may ask whether you want to take the toll road ("jalan toll"). I'd say yes! The driver will expect you to give him the 10 000 Rp amount of the toll when he gets to the toll booth. This will save you at least 10 minutes on the trip, and double that if you arrive in late afternoon like we did. If the driver does not ask, then I'd make the suggestion myself.
For the return trip, Prima Cottage arranged for a driver, but we could also have easily found a taxi.
