Having been a hotel hound for the last 3 months, I have seen a lot of these mistakes. Usually it is caused by a combination of mistakes. One is doing too much too fast, another is a new person on staff but I have found one of the most common problems is just plain laziness, oh and never underestimate the power of cheep. Good luck and I hope the rest of your trip goes much better.
