Most (all?) of the boats leave from Bang Bao pier. There's a slow old fishing boat (Bang Bao Boat) and faster speedboats. You can book the ticket at any travel agent on Trang. If you want the cheaper boat ask them for it otherwise they might sell you the speedboat by default.

The boats stop off at Koh Mak, which is worth at least 1 night in my opinion.

Same trip in reverse, or if you prefer you can get the boat direct to the mainland (Laem Sok). Any agent on Koh Kood will be able to sell you that ticket and will probably be able to route you all the way through to BKK.

The Kai Bae Hut speedboat is a better option if you are staying on White Sand, Klong Prao or Kai Bae beaches - as it saves the pick up truck ride down to Bangbao. Ticket price is 900 Baht / person, the same as Bangbao boat. Buy tickets from any tour agent when you are on the island.

You can buy a combined boat and bus ticket from Koh Kood to Bangkok through Boonsiri Ferry. See http://www.boonsiriferry.com/timetable - 850 Baht total. Buy online or from your hotel when you are on Koh Kood.
