You can get a car from St. Thomas at the airport and then go to Red Hook (where the ferry docks are) and take a car ferry over. If you are renting from the airport call BUGET, HERTZ, AVIS or someother car rental before departure so you can guarantee a car when you get there.
The Red Hook ferry is very difficult to find. You can easily pass it up for there is no sign to the ferry dock. Red Hook dock is under construction (6/1/2006) and will be much more user friendly when they complete the construction (in a few years maybe, nothing happens fast on the islands). When you get to the dock, you must back your car in. That is the way you will board the car ferry. Don't try to take cuts as the islanders will get upset if you do. First on the ferry = last out. The ride from the airport to Red Hook is about 25 minutes. However, allow 45 minutes if going during rush hour.
Renting a Car-St. John
There are several car rental companies right in Cruz Bay where the ferry stop is on St. John. They are all within walking distance of the dock (heck, the whole town is within walking distance of the dock!). Check this
